Same-Day Medical Access for Established Patients

In addition to our regular appointments, we also have several same-day appointments available for established patients. If you have established care with one of our providers and need same day access to care, call first thing in the morning at (406) 258-4789, and we will make every effort to get you an appointment that day.

If you are new to PHC and need same day care, call us and we will do our best to get you in as quickly as possible. It’s helpful if you fill out your forms before your appointment. 

For after hours care, call (406) 258-4789 and you will be connected with our on call provider.

What to Expect

We are committed to serving you as a patient-centered medical home. Your health care at Partnership Health Center focuses on your health goals, and you will work with a medical team to help you reach those goals.

If you are seeking behavioral health services or dental care, we are equally committed to serving your needs as a team. Your dental care team includes your dentist, a hygienist, dental assistants, and reception.  We also offer behavioral health as a standalone service or as an integrated part of your medical care. 

New Patient Registration Forms (Required)

New patients are required to fill out a few important forms before their first appointment. New patients are welcome to fill out these forms with either option below: 

  • At a PHC clinic right before your appointment: Please arrive 15 minutes early for your appointment so you have enough time to fill out your forms
  • Ahead of time: Filling out your forms ahead of time can help make the registration process faster and easier! Visit our registration forms page to find the forms you need for your appointment.
  • Need help? Give us a call at 406.258.4789. Our friendly staff will be happy to answer any of your questions. 
Sliding Fee Discount

Our services are available on a Sliding Fee Scale (SFS). The SFS adjusts costs for services based on your household income and size. This is available to everyone who qualifies, even people who have insurance. 

*IMPORTANT:  To apply for a Sliding Fee Scale discount, please submit or bring in a photo ID, your insurance card, and your proof of income.

Insurance Assistance

Applying for health insurance can be confusing. At PHC, we make it easy with free, one-on-one assistance. 

Call (406) 258-4450 to speak with one of our Certified Application Counselors! Appointment available upon request. We’ll help you review your insurance options and help you with your insurance applications.  We can also help with Medicaid applications.

Your Health History

The more we know about your health history, the better we are able to serve you – the whole you! Please tell us about any medications you are taking and provide records of any previous treatment you have had at other facilities. 

Patient Portal

Once your registration has been completed, we can give you access to our patient portal. This is a secure, web-based service. Once you are enrolled in the patient portal, you can use this tool to: 

  • Communicate with your care team
  • Request appointments
  • View your health records
  • Request pharmacy refills
