What is Primary Medical Care?
At PHC, most of our medical team is trained in Primary Medical Care, also known as Family Medicine. Family Medicine is a specialty focused on meeting patient health needs at all stages of life.
Our Primary Care Providers are dedicated to partnering with patients to help meet their short and long-term health goals. When you visit PHC, your Primary Care Provider will listen to you carefully to understand the full picture of your health and create a treatment plan that meets your unique needs.
Your Primary Care Provider is the first stop on your medical journey. When you have a question or concern about any aspect of your health and wellbeing, your PCP is your go-to resource for treatment, support, and guidance for next steps. If your medical need requires a specialist outside of PHC, our team will provide a referral and help make sure you’re connected to the care you need.
Affordable Healthcare for All
We believe that cost should never be a barrier between you and quality healthcare. We use a sliding fee scale to adjust charges for healthcare services based on your household income, and nobody is turned away from PHC due to inability to pay. Call us to learn more about how we can make sure great healthcare is affordable and accessible for you and your family.
Whole-Person Healthcare
The different aspects of your health are interconnected. We believe your healthcare should be too.
Traditional healthcare models require you to go to one place for your dental health, another for your physical health, another for your mental health, and so on. At PHC, our Whole-Person Care Model means you can access all of these services under the same roof. You can even fill your prescriptions in our on-site pharmacy.
Our Whole-Person Care Model also means that when you access multiple services at PHC, our expert team works together to carefully consider the ‘big picture’ of your overall health and wellbeing. When our array of healthcare professionals combine their knowledge and expertise, it creates better health outcomes and a more seamless experience for our patients.
Integrated Behavioral Health
Integrated Behavioral Health is available for all of our medical and dental appointments.
This means a behavioral health provider or social worker can join any medical or dental appointment to help answer questions and provide support. To access this service, simply ask to connect with a behavioral health provider during your next medical or dental appointment. Our team will come to you.
We can support patients with things like:
- Stress
- Depression
- Anxiety
- Substance use
- Reaching lifestyle goals
- Parenting support
- Housing
- Transportation
- Translation
- Paying for medication
- Paying heating or rent
- And much more!
