During any appointment at PHC, simply ask to connect with a Behavioral Health provider and a member of our team will connect with you during that same appointment.
Individual Therapy
Individual therapy appointments are one-on-one meetings with a compassionate behavioral health clinician.
Your provider will listen to you carefully and can help you address concerns such as stress, anxiety, depression, substance use, relationship challenges, reaching lifestyle goals, and more. Individual appointments are available in-person or through telehealth.
Telehealth
Accessing therapy should be easy. Telehealth Behavioral Health appointments mean you can access care from a location that is most convenient for you. All you need is access to the internet and a device such as a smartphone, tablet or computer.

At these sites, PHC staff can also connect families to medical care, dental care, health insurance, food resources, and housing resources.
Current schools include: Lowell Elementary School, Franklin Elementary School, C.S. Porter Middle School, and Willard Alternative High School.
If you have a child at one of these schools and would like to learn more, please contact the front office at your school.

Our Model of Care
For too long, healthcare has focused on one aspect of your health at a time: your teeth, your body, your mind, and so on. But at PHC, we know that these different parts of your health are linked to one another. That’s why we offer a patient-centered, whole-person care model that considers all aspects of your health and wellbeing.
When you access multiple services at PHC, our expert team works together to ensure your care is carefully coordinated to best support you and your unique needs. That means that your doctor, therapist, pharmacist, dentist (and many other members of our staff) will team up to create a care plan that considers all aspects of your health and makes your patient experience as seamless and stress-free as possible.
Cost should never be a barrier to care. We use a sliding fee scale to adjust charges for care based on your household income (in other words, if you make less, you pay less.) Call us at (406) 258-4789 or click the link below to learn more about our commitment to serving all people, regardless of ability to pay.
At PHC we offer Integrated Behavioral Health for all of our medical and dental appointments, which means a behavioral health provider or social worker can join medical or dental appointments to help answer your questions and provide support. To access this service, simply ask to connect with a behavioral health provider during your next medical or dental appointment. Our team will come to you.
We can support patients with things like:
Stress | Housing |
Depression | Transportation |
Anxiety | Translation |
Substance use | Paying for medication |
Reaching lifestyle goals | And much more! |
*No-Show and Cancellation Policy
Because behavioral health appointments are in high demand, we request that you give at least 23 hours’ notice if you need to cancel or reschedule. We find for patients with consistent appointment no-shows or cancellations it is best to move to a same-day appointment. Your healthcare team will discuss this with you if this option appears to be the right fit.
