To re-apply, simply turn in recent proof of income for each earner in your household. Our definition of household is on this page.

Here is a list of what we can accept:

  • MOST RECENT 1040 TAX FORM (PHC’s preferred documentation)

  • ONE MONTH’S RECENT, BACK-TO-BACK PAY STUBS OR UNEMPLOYMENT CHECKS (for all employment if working multiple jobs)

  • SOCIAL SECURITY OR DISABILITY BENEFIT LETTER (or most recent bank statement showing monthly deposits)

  • RECENT BANK STATEMENT (for fixed incomes)


  • GRANT AWARD LETTER (students only)

  • SCREENSHOT OF WIC BENEFITS (from the WIC Shopper app)

Please be aware that we cannot accept W-2’s, 1099s, old taxes, or student loan information.

Does anyone else in your household come to PHC? If so, let us know. We’d like to make sure that your sliding fee scale discount gets applied to everyone’s accounts.

How to turn in your proof of income:

  • Mail or fax your information. Be sure to put your name and date of birth on it so we know that it is for your account.
  • If you would like to turn your information in online, go to the Forms page on our website. Open and submit a “Sliding Fee Scale Application.” You will be able to attach your documents.

Please don’t wait! If you bring in your information as soon as possible, we will see if we can go back and discount any recent visits.

What is PHC’s Definition of “household?”

A household includes all individuals who live together and are related by birth, marriage, or adoption. It also includes all individuals who may or may not live together, but share a taxed household.

What's next?

When we bill for your visits, we will bill your insurance first. Next, we apply your sliding fee discount to the remaining balance. If you do not have insurance, we apply the discount to the full charges. Individuals and families who qualify for a Slide E may still receive some discounts on prescriptions in our pharmacy. Proof of income is required.

How do I set up a flexible payment plan?

We don’t expect you to pay for services upfront or all at once. We also don’t have a minimum payment amount. We are happy to put together a payment plan that fits your budget. Any regular payment activity on your account will keep you in good standing. This way you won’t have to worry about collections.

How do I make a payment?
  • Your statement will have information on how to log into our payment system by creating a login, or by using the Guest Pay.
  • You can pay online through the Pay My Bill page.

What if I need assistance getting insurance coverage?

We have Certified Application Counselors (CACs) that can help you apply for health coverage. Call (406) 258-4450 to speak to a CAC. Our CAC’s can help you with Medicaid and Marketplace applications. Let’s get you covered!

What about dental procedures?

Some dental treatments like root canals and multiple extractions will require a down payment before you are seen, or at the time of check-in. Dental Appliances are also discounted at a different discount level.

Are all services discounted on the same level or on the sliding fee scale?

​These discounting on an alternate schedule are OB and special lab tests.  DOT physicals do not discount. We are happy to tell you more about the specifics for non-discounted services. Call for more information.
