Dear Patient,

You are receiving this letter because you were recently enrolled in Partnership Health Center’s Patient Centered Medical Home (PCMH) Program through Medicaid. This program is designed to help patients access health care through one clinic with the help of a Nurse Care Manager. We know the health care system can be confusing; our Nurse Care Manager, Tracy Bennett, can help you connect with your care team and answer any of your questions.

As a Medicaid patient, you can choose from several different medical providers who accept Medicaid.  Currently, you have been assigned to Partnership Health Center. We would be honored to continue as your health care provider. If you haven’t seen us before, we would be thrilled to work with you to meet your health care needs. Our team provides a wide range of patient-focused services including:

  • Primary Medical Care – A team of doctors and nurses to address medical needs for patients of all ages.
  • Dental – Dentists and dental hygienists for routine dental needs and emergency care.
  • Retail Pharmacy – Prescriptions filled on site with options for reduced costs.
  • Clinical Pharmacy – Expert medication review for health issues such as diabetes, asthma and high blood pressure.
  • Behavioral Health Services – One-on-one or group therapy for mental and emotional health support.
  • Social Work – Connect with resources for housing, food assistance, legal issues and more.
