Sports Physicals at PHC
Here at PHC, we can complete a sports physical during your child’s yearly wellness exam with no additional cost added to your visit. These well child checks are a great time for you and your child to meet with their primary care provider, ask questions, and address any concerns you may have. It is also a great opportunity to double check that your child is up to date on their teenage immunizations including: TDaP (tetanus, diphtheria and pertussis), HPV (human papilloma virus), and Meningococcal (meningitis) vaccines.
Prior to school starting again in the fall, you may also call to schedule an appointment with us for adolescents to come in and complete their sports physical exams. Your insurance will be billed for the visit but if it is not covered by insurance, you will be billed a $25 flat rate.
Please call our clinic at (406) 258-4789 with any questions or to schedule your well child check appointment. Also, please print, fill out, and bring this document with you to the sports physical appointment. If you’re unable to print beforehand, we will provide a copy for at your appointment.
If you are sending your child under 18 to be seen alone, you will need to fill out a parental consent form here.
