Dear Partnership Health Center Patient,

Now that you are in young adulthood, it is time for you to make the transition out of pediatrics and into family or adult medicine. This is an exciting transition and also one that takes some thought.

Partnership Health Center and I recognize that your choice of a health care provider is a personal decision. I would be happy to review options with you after considering your situation and your preferences. If needed, I am also happy to meet with you for a last appointment to ensure you are feeling ready.
