It looks like you did not make it to your last appointment at PHC.  We are sorry we missed you and we hope you are well!  If you missed your visit because you are having trouble getting to your appointment, we have a few solutions.  We can see if our social work team can help you with transportation, or we can check to see if a phone or video appointment would work for you and your provider.

We understand that life is complicated and there are many reasons you may have missed your appointment.  Our primary goal with this notification is to make sure you get reconnected to the care you need and deserve.  Please call to reschedule your appointment and explore your options as soon as possible.
