Greetings from Partnership Health Center!

I am writing to let you know that I will be leaving Partnership Health Center as of October 29, 2024. I plan to discuss this personally with as many patients as possible before leaving, but also wanted to ensure that you have written information regarding this transition and can take the next best steps for you and your care.

I recognize that your choice of a behavioral health clinician is an individual and personal decision that only you can make. Please feel free to give us a call at (406) 258-4789 and request to schedule with a new Behavioral Health Clinician. I can also recommend additional options for you after considering who you are, your unique situation, and the nature of the behavioral health provider that would likely be the best “fit” for you.

It has been an honor to serve my patients at PHC, and I am grateful to everyone that has entrusted me to care for them and share in decisions about their health.

Frequently Asked Questions

My provider is leaving, what do I do now?

First, remember your Care Team is still here for you! We have an entire PHC Team including a number of other providers that can help with your care.

Will the Care Team know me?

Yes, the Care Team and covering provider will have your information. All of your health records are in our system.

What do I do about medication refills?

As always, start with your pharmacy. They will contact us, but be aware that using the automated refill request systems may cause a brief delay in getting your medication.

How do I get the results of tests ordered by my provider?

Another provider on your Care Team will still receive these results. If you have had tests done and have not heard about the results within 5-7 days, please contact us.
