Dear Patient,
Greetings from Partnership Health Center!
I’m writing to share that I will be leaving my position at Partnership Health Center at the end of July 2025. It has truly been a privilege to be part of your care and to walk alongside you on your journey. Please know that this decision was not made lightly, and I am deeply grateful for the trust you have placed in me.
As PHC works through this transition, I want to acknowledge that there may be some changes in how psychiatric care is provided. Right now, we have one remaining psychiatric provider, and while the team is working hard to expand access, appointments may be limited in the short term.
That said, you are not alone. The care team at PHC is here for you and will continue to support your needs in the following ways:
- They will do their best to schedule you with another provider as availability allows.
- If internal appointments are limited, the team can help refer you to community resources for continuity of care.
- If you have questions about your medications, need refills, or need support during this time, please don’t hesitate to call your medical care team.
While I will no longer be part of your care team, I leave knowing you are in the hands of compassionate and dedicated professionals. Thank you again for allowing me to be part of your care. I wish you continued strength and wellness in all the days ahead.
Warmly,
Rachel Haase

Yes, the Care Team and covering provider will have your information. All of your health records are in our system.
As always, start with your pharmacy. They will contact us, but be aware that using the automated refill request systems may cause a brief delay in getting your medication.
