Our staff understand that older adult patients may have more complex needs. Every member of our team is dedicated to providing attainable, high-quality care with grace and kindness at heart.
Services
In addition to all of the many services we provide to patients of all ages at PHC, we provide several services designed specifically for older adults.
Medicare Wellness Visit
- This annual visit is an important benefit of Medicare Part B that focuses on prevention. It is not a physical exam that you typically receive from your provider.

End-of-Life Planning
- Do you know who will make health care decisions for you if you are unable to make them for yourself? We recommend that all patients talk to their loved ones about what matters most to you if you get seriously ill. This is called advance care planning and it ensures that your wishes are documented in an Advance Directive like MyChoices or Five Wishes (available at your appointment).
