Greetings from Partnership Health Center! I hope this letter finds you well in the New Year.
I am writing with some news that is certainly very hard to share. I will be leaving my position with Partnership Health Center (PHC) in June 2025. My family and I will be moving and, sadly, we will be leaving the Missoula community. While this next step in our family’s life is an exciting time, it is still very bittersweet in the goodbyes it involves, especially with my patients. I have been honored to serve as your Family Physician, and for some of you, that has spanned the 10+ years that I have been with Partnership Health Center. It has been a sincere privilege to serve as your physician, and I truly value the relationship I have had with you over time.
In the meantime, I will be scheduling visits through mid-to-late May 2025. I understand that this transition might cause some uncertainty or stress in the process of switching to a new primary care physician. We have an excellent team of providers at Partnership Health Center who will be available to you at the Creamery location for coverage. The clinic will work to assign a new PHC provider for your primary care after my departure, and of course, if you are already familiar with one of my colleagues who is accepting new patients, please feel free to request assignment to them if you prefer.

First, remember your Care Team is still here for you! We have an entire PHC Team including a number of other providers that can help with your care.
Yes, the Care Team and covering provider will have your information. All of your health records are in our system.
As always, start with your pharmacy. They will contact us, but be aware that using the automated refill request systems may cause a brief delay in getting your medication.
Another provider on your Care Team will still receive these results. If you have had tests done and have not heard about the results within 5-7 days, please contact us.
